We designed ClearPath as a coordination hub that automates and tracks every step of the discharge process. The platform uses NLP and ML to analyze patient records, monitor pending tasks, and predict discharge timelines. It notifies medical staff when actions are needed, and gives patients a clear view of what remains before they can go home.
